In our institute, students who could not attend the Midterm Exams due to their excuses in the 2020-2021 Academic Year Spring Semester must fill out the Excuse Form available at the address https://prg.mehmetakif.edu.tr/MazeretFormu by 24:00 on May 2, 2021.
Our students are required to log in to the above address with their student number and student information system (ÖBİS) passwords, check and approve the information that appears, and follow the steps below:
Ø They should enter the course and course instructor information and click the “Add Course Information” button,
Ø They should briefly state their reasons for not being able to attend the Midterm Exam,
Ø If they have mandatory excuses that need to be documented, they should upload the relevant documents (medical report, death certificate, etc.) in PDF format,
Ø After filling in the fields marked with (*) completely, they should click the “Submit Application to Unit” button to complete the process.
The specified application deadlines will not be extended under any circumstances, and all responsibility regarding the matter lies with the student.
